Contacting & Visiting Michael's Practice

For Patients: Dr Wyatt is able to see patients at at the Mayo Specialist Centre in Taree & the The Dolphin Suite Consulting Rooms in Forster. For your assistance we have provided maps showing the location of both facilities below. Michael's friendly staff are here to help & they look forward to assisting with any questions that you may have. To make it convenient for you to contact the practice we have provided a patient specific enquiry form below.

If you already have an appointment with Dr Wyatt you may be wondering what to bring with you? Please make sure that you have your GP referral & any relevant X-Rays that might be in your possession. It is also important to bring a list of all medications you are currently taking. It's also a good idea to come to the practice wearing comfortable clothes. If you have a Medicare & Private Healthcare Card please have this with you if possible.

Payment for the consultation at Mayo, Dolphin Suite (Forster), or Lingard is due at the time of the appointment. This can be made with either a Debit Card or Credit Card.
